Fix prompt truncation for non-ASCII lines
The prompt was previously assuming that each grapheme cluster in the
line was single-width and single-byte. Lines like the one in the new
integration test would cause panics because the anchor attempted to
slice into a character.

This change rewrites the anchor and truncation code in the prompt to
account for Unicode segmentation and width. Now multi-width graphemes
can be hidden by multiple consecutive elipses - for example "十" is
hidden by "……" (2-width).

We should skip zero-width graphemes and reset only long (more than
1-width) graphemes.

* helix-tui: Use zero-width-space for zero-width grapheme test

The update of unicode-width 0.1.13 in the parent commit changed the
width of the U+1 codepoint to 1 from 0, causing the test to fail. We
can switch to a well known zero-width codepoint of U+200B to fix the
behavior.

Performant and correct set_spans_truncated
the previous implementation used set_string_truncated. This is not only
awkward with this kind of "streaming" string (and therefore lead to an
inefficient and incorrect initial implementation) but that function also
truncates strings of width 1 when there is only a single char available.

The implementation here is performant, correct and also handles the
single width case correctly.

make underline_style a seperate option
Underline styles are mutally exclusive and overwrite each other.
Therefore implementing as an modifier lead to incorrect behaviour
when the underline style is overwritten.

For backwards compatability the "underline" modified is retained (but
deprecated). Instead the "underline_style" and "underline_color"
optios should be used to style underlines.
